Marshall University Survey - barriers to care for individuals with chronic pain

Greetings, 

You are invited to participate in a survey of health care providers regarding training and experience in working with individuals with pain, perspectives on pain care and behavioral health treatment, and potential barriers to referring patients to behavioral health treatment. The goal of this study is to learn more about the barriers to care that health care providers have identified in order to develop programs to address barriers and improve access to care in the future. Your input as a health care provider is important in understanding how we can improve access for patients with chronic pain conditions. The survey will take approximately 15-20 minutes to complete. To thank you for your participation, you will have an opportunity to enter a drawing for 1 of 4 $25 Amazon.com giftcards at the conclusion of the survey if you choose.
